All Adult Clinics will be held at Cheverus Tennis Complex this year unless otherwise stated. Please circle the program dates you would like to take part in & mail this page in with your payment. Remember you must sign up and pay for the whole month at a time when registering.

    STATEMENT OF AGREEMENT
    Unless otherwise stated, my signature permits you to use photographs for camp purposes. Grand Slam Tennis Camp will safeguard the health of each participant but will not be responsible for sickness or accident. The applicant is in good health, is covered by insurance and is able to participate in physical activity. I authorize the staff members to act for me, according to their best judgment in any emergency requiring medical attention.
